Key takeaways
- The Botox Cosmetic label gives 3 to 4 months for frown lines.
- Results fade gradually: about half of people still looked improved at 3 months and a quarter at 4 months.
- The 6-month figure comes from Daxxify and from Botox for underarm sweating, not Botox for wrinkles.
- A 20-unit dose lasted longer than 10 units in one study, 17.1 weeks against 14.8 weeks.
- Treatment more often than every 3 months has not been tested for safety or effectiveness.
How long do Botox results last on average?
The Botox Cosmetic label gives 3 to 4 months for frown lines, the area it was tested on.
Cleveland Clinic and the American Academy of Dermatology give the same 3 to 4 months.
The results fade gradually because Botox works by blocking the nerve signal that tightens the muscle, and the label explains that the muscle slowly regains that signal.
In the label's trials, 405 adults had a single 20-unit dose for frown lines.
Investigators rated their frown lines as none or mild at a full frown in 80% of people at day 30, 70% at day 60, 48% at day 90 and 25% at day 120.
So about half of people still looked improved at 3 months and a quarter at 4 months.

The 6-month figure you often see online comes from a different toxin, Daxxify, and from Botox for underarm sweating, not from Botox for wrinkles.
How long does Botox last by area?
The label only states a duration for frown lines, the lines between the eyebrows.
For other cosmetic areas there is no label duration, so any month count you see for the forehead or crow's feet is an estimate, not a tested figure.
Sweating treatments are the exception, because they have their own data.
In the Botox trial for underarm sweating, the median response after the first treatment lasted 201 days.
The American Academy of Dermatology lists 3 to 10 months for underarms or hands, 3 to 6 months for feet and 4.5 months for the face.
How long Botox lasts, by use
| Use | Typical duration | Source |
|---|---|---|
| Frown lines | 3 to 4 months | Botox Cosmetic label |
| Sweating: underarms or hands | 3 to 10 months | AAD |
| Sweating: feet | 3 to 6 months | AAD |
| Sweating: face | 4.5 months | AAD |
What makes Botox wear off faster?
Dose is the clearest factor in the studies.
In a 64-week study of 363 people, a 20-unit dose for frown lines lasted 17.1 weeks on average, while a 10-unit dose lasted 14.8 weeks.
Sex and age matter too.
In the label's trials, 85% of 334 women responded at day 30, compared with 59% of 71 men, and responses looked lower in people 65 and older.
The label does not tell providers to give men a higher dose.
Exercise and a "fast metabolism" are often blamed for Botox wearing off early, but we found no label or controlled trial that shows it.
If Botox seems not to work at all, the labels name the dose, the injection technique and, rarely, antibodies to the toxin as possible reasons.

Does Botox last longer after repeat treatments?
The evidence is mixed.
In the 64-week study of 363 people, the duration did not change with repeated treatments.
A small 2026 study of 21 women treated regularly for 2 years found that muscle activity stayed suppressed even after treatment stopped, and its authors called for larger studies.
The often-quoted idea that Botox prevents wrinkles rests mostly on one pair of identical twins, in which the twin treated regularly for 13 years had fewer lines at rest.
That is one case comparison, not proof.
When should you book your next Botox appointment?
The label says treatment more often than every 3 months has not been tested for safety or effectiveness.
Repeat treatment can also lead to antibodies against the toxin, which can make it work less well.
In the crow's-feet trials, 1.5% of 916 treated people developed binding antibodies and none developed neutralizing antibodies.
The label advises the lowest dose that works, given at the longest practical interval.
A licensed provider can tell you what interval suits your dose and area.
Frequently asked questions
Does Botox last longer in men or women?
The Botox Cosmetic label trials found higher response rates in women than in men at the same 20-unit dose: 85% of 334 women responded at day 30, compared with 59% of 71 men.
A 2010 review of duration studies reported frown-line effects returning to baseline after 3 to 5 months in women and 4 to 6 months in men.
The label does not recommend a different dose for men.
How long does Botox for sweating last?
Longer than Botox for wrinkles.
In the Botox trial for underarm sweating, the median response after the first treatment lasted 201 days.
The American Academy of Dermatology lists 3 to 10 months for underarms or hands, 3 to 6 months for feet and 4.5 months for the face.
Why did my Botox wear off after a month?
The labels name the dose, the injection technique and, rarely, antibodies to the toxin as reasons a treatment may work less well than expected.
Lower doses wear off sooner: in one study a 10-unit dose for frown lines lasted 14.8 weeks on average, against 17.1 weeks for 20 units.
Ask your provider how many units you received.
