Biggest med spa chains by number of locations
The figures below are the location counts each chain publishes on its own website, read in October 2026, rather than figures from third-party location databases, which in our research ran behind the chains' own counts.
| Chain | What it says about its size | Main services |
|---|---|---|
| Milan Laser Hair Removal | Over 400 locations nationwide | Laser hair removal |
| Restore Hyper Wellness | 210+ studios nationwide | IV drips, cryotherapy, infrared sauna, red light therapy and compression |
| LaserAway | Over 200 locations nationwide | Laser hair removal and injectables |
| Sev Skin (formerly SEV Laser) | More than 70 studios; its own location selector listed 77 studios in 15 states, 28 of them in California | Laser hair removal |
| VIO Med Spa | Over 64 locations in 20 states | Med spa services; sells franchises |
| SkinSpirit | Over 50 locations, in Arizona, California, Colorado, DC, Maryland, North Carolina, New York, Oregon, Texas, Utah and Washington | Injections |
| Ideal Image | Could not be confirmed (see the note below) | — |
Each figure is a chain talking about itself, and each is stated as a "more than" number, so treat the table as a size guide rather than an exact ranking — the counts move as locations open and close, and they read differently depending on who counted.
Restore's own disclaimer shows why "chain" needs unpacking: Restore says its studios are individually owned and operated franchises and that medical services are provided by an independently owned physician practice.
For scale, AmSpa's 2024 State of the Industry survey estimated 10,488 US medical spas in 2023, up from 8,899 in 2022, and found 81% single-location.
AmSpa's public summary does not say how it counted individually owned franchise studios, so that figure does not tell you how many med spas belong to chains.
When we checked on October 16, 2026, idealimage.com showed only a registrar "coming soon" parking page, so Ideal Image's operating status and location count could not be confirmed from its own site. Florida's WARN layoff-notice list shows a notice from Ideal Image dated December 22, 2025 for 255 employees at its Tampa headquarters, with a layoff date of February 20, 2026.
Laser hair removal chains: LaserAway, Milan Laser, Ideal Image, SEV, Simplicity, Satori
Laser hair removal is the anchor service for this group, and two framing points come straight from the chains and the device rules.
Hair-removal lasers are 510(k)-cleared devices, and their cleared claim is permanent hair reduction — fewer hairs regrowing over the long term — not removal of all hair.
LaserAway, Sev Skin and Milan describe their lasers as FDA-cleared — a 510(k) clearance, which is different from FDA approval.
LaserAway says it has over 200 locations nationwide.
Its pricing page (read October 16, 2026) lists laser hair removal at $99 to $649 per treatment by area, in single sessions or packages of 3, 8 or 10, and states that prices may vary by location.
LaserAway also says every treatment is performed by a registered nurse, nurse practitioner or physician associate, that clinics are overseen by board-certified dermatologists (the chain does not name the board), and that it uses no estheticians or technicians; it calls itself an "aesthetic dermatology clinic—not a med spa."
Milan Laser says it has over 400 locations nationwide; its own about page gives 38 states and its FAQ 37, which is why we state only the location count.
Milan does not publish package prices online — it gives personalized quotes and offers installment plans that it says start at 0% APR, possibly through third-party lenders.
Its "Unlimited Package" comes with every body area purchased: Milan describes it as unlimited treatments for life on that area and says most clients need 7 to 10 treatments to be "up to 95% hair free," while Milan's own disclaimer says "permanent results" does not necessarily mean the elimination of all hairs.
SEV Laser rebranded as Sev Skin in 2026; the company says it was founded in 2010 as a laser hair removal practice inside a hair salon.
Sev Skin says it has more than 70 US studios, that laser hair removal memberships start at $15 per month and single sessions start at $29, with prices varying by treatment, area and location, and that it advertises a free first treatment on select areas for clients new to a service.
The company says its treatments are performed by nurse practitioners, registered nurses, licensed estheticians and laser specialists using FDA-cleared devices.
What three laser chains publish
| Chain | Published pricing | Terms the chain states |
|---|---|---|
| LaserAway | $99 to $649 per treatment by area | Single sessions or packages of 3, 8 or 10; prices may vary by location |
| Milan Laser | No package prices online | Personalized quotes; installment plans it says start at 0% APR |
| Sev Skin | Memberships from $15 per month; single sessions from $29 | Prices vary by treatment, area and location |
Ideal Image sits in this group too, but as the note in the section above explains, we could not confirm its current operating status or location count, so it has no row of its own beyond that caveat.
Simplicity Laser and Satori Laser complete the group, and we have not yet verified facts about either chain's locations or prices; rather than quote numbers we cannot source, we leave those details to each chain's own website.
For how many sessions laser hair removal takes and what it involves outside the chains, see our laser hair removal guide.
Injectable and facial bars: Ever/Body, Alchemy 43, Ject, Peachy, Plump, SkinSpirit, Skin Pharm, Glowbar, Heyday, Skin Laundry
We group these chains as injectable and facial bars; the grouping is ours, not a confirmed description of each chain's menu.
SkinSpirit is the one our research has verified so far: it says it has over 50 locations nationwide, in Arizona, California, Colorado, DC, Maryland, North Carolina, New York, Oregon, Texas, Utah and Washington.
SkinSpirit says injections are performed by registered nurses, nurse practitioners and physician assistants overseen by a medical director, and that it accepts CareCredit and Allē payment plans powered by Cherry.
Its published cancellation policy requires 24 hours' notice for appointments under 2 hours and 48 hours for longer or group appointments, charges $75 to $300 for late cancellations and $150 to $300 for no-shows, and gives store credit only — within two weeks — for retail returns.
Ever/Body, Alchemy 43, Ject, Peachy, Plump, Skin Pharm, Glowbar, Heyday and Skin Laundry are also in this group, and we have not yet verified location counts, menus or policies for them; their own sites are the current source for those details.
Cancellation, no-show and package-expiry terms are set by each chain, and fees like SkinSpirit's can be significant. The chain's own policy page is the authoritative version for the location you book.
Full-service med spa franchises: VIO, Evolve, dermani, 4Ever Young, Sona, Elase, It's A Secret
We group these as full-service med spa brands; the grouping is ours, not a confirmed description of each brand's menu or ownership.
VIO Med Spa is the one our research has verified so far: it says it has over 64 locations in 20 states, that a board-certified plastic surgeon leads its clinical direction (VIO does not name the board), and that it sells franchises under "Own a VIO."
The company says its treatments are provided by physicians, nurse practitioners, physician assistants and registered nurses, and that the same provider who assesses you performs the treatment.
VIO's ClubVIO membership, as published when we read it in October 2026: the Signature tier costs $99 per month and banks that amount toward treatments, with $2 off each toxin unit and 10% off products and services; the Elite tier, priced by location, gives $3 off per toxin unit and 15% off; and a Founding Member tier at $99 per month with Elite benefits is limited to the first 150 members.
ClubVIO Signature
- $99 per month, banked toward treatments
- $2 off each toxin unit
- 10% off products and services
ClubVIO Elite
- Priced by location
- $3 off per toxin unit
- 15% off
Evolve, dermani, 4Ever Young, Sona, Elase and It's A Secret are in this group too, and we have not yet verified facts about their locations, ownership or prices.
IV, wellness and hormone chains: Restore Hyper Wellness, Prime IV, The Hydration Room, Hydrate IV Bar, Ageless Men's Health
Restore Hyper Wellness is the chain our research has verified in this group: it says it has 210+ studios nationwide and that its studios are individually owned and operated franchises, with medical services provided by an independently owned physician practice.
Restore says founder Steve Welch opened the first studio, built around cryotherapy, in Austin, Texas in 2015, and that it began franchising with multi-state expansion in 2017.
Its service list includes IV drips, cryotherapy, infrared sauna, red light therapy and compression, and Restore says members get 30% off special services and packages such as facials plus two monthly guest passes.
Membership terms matter here: Restore says its monthly memberships carry a 3-month minimum commitment and then auto-renew monthly until cancelled, credits on most plans expire 100 days after issuance (Core plan credits expire monthly), and packages and first-month promotions must be used at the studio where they were bought.
Prime IV, The Hydration Room, Hydrate IV Bar and Ageless Men's Health complete the group, and we have not yet verified facts about their locations or offerings.
If you are researching the treatments themselves, our IV therapy guide covers what an IV drip is and how it is priced, and our hormone replacement therapy guide covers hormone treatment.
This page lists services as the chains list them; it does not evaluate them. Claims that IV drips "detox," "boost immunity" or cure hangovers are not established fact — the FTC has acted against such claims, and the evidence behind them is limited.
Chain vs independent med spa: what changes for you
Start with what a "chain" even is: there is no single federal legal definition of a med spa, and whether one is regulated as a medical practice, and who may own it, is determined by each state's rules (our what is a med spa guide covers the basics). "Chain" is a business shape, not a legal category — Restore describes its studios as individually owned and operated franchises and VIO sells franchises, so the same word covers different setups — and AmSpa explains that many states bar non-physicians from owning a medical practice directly or through a business entity (the corporate practice of medicine doctrine), with management-services organization (MSO) arrangements commonly used where ownership is restricted.
Price is the clearest practical difference, and it is mostly about transparency: some chains publish menu prices — LaserAway's pricing page (read October 16, 2026) lists Botox at $15 per unit, or $10 per unit in a 300-unit bank, and Juvéderm lip filler at $450 per full syringe — while others, like Milan, give personalized quotes only.
LaserAway and Sev Skin both state that prices vary by location, so a published menu is not the price at every address, and promotions move too: LaserAway was running a time-limited "70% off laser hair removal" promotion when we read its page in October 2026.
Whether a chain or an independent is cheaper for the same treatment is not something our research establishes — compare the dated menu against local quotes.
Several chains sell memberships: Sev Skin's laser hair removal memberships start at $15 per month, ClubVIO's Signature tier is $99 per month, and Restore's memberships carry a 3-month minimum and then auto-renew.
On auto-renewal, the FTC's 2024 "Click-to-Cancel" rule was vacated in full by the Eighth Circuit on July 8, 2025, before it took effect, so no FTC click-to-cancel rule currently governs med spa memberships; state auto-renewal laws and case-by-case FTC Act enforcement still apply.
Chains publish staffing descriptions, and they differ: LaserAway says RNs, NPs or physician associates perform every treatment under dermatologist oversight it describes as board-certified (board not named); Sev Skin says NPs, RNs, licensed estheticians and laser specialists treat clients; SkinSpirit says RNs, NPs and PAs do injections under a medical director.
Whoever signs the paycheck, the injection rules are the same as everywhere else: AmSpa says injecting botulinum toxin is the practice of medicine because a prescription drug is used on living tissue, so it must be done by a physician, an independent-practice NP or another licensed professional acting within their scope, under rules that vary by state.
AmSpa likewise describes the "good faith exam" it says should come before a first treatment anywhere: a physician, PA or advanced practice nurse takes a medical history, does an appropriate exam and creates the treatment order.
Our state-by-state med spa laws guide covers who may inject and supervise where you live, and our guide to choosing a med spa turns the vetting into a checklist.
On reviews: we don't publish review scores for chains, and review-site ratings deserve a caveat of their own.
The FTC's Consumer Reviews and Testimonials rule (16 CFR Part 465) bans fake reviews, paying or giving incentives for reviews that express a particular sentiment, undisclosed insider reviews, company-controlled "independent" review sites, and suppressing negative reviews through threats or by hiding reviews based on rating.
The rule was announced August 14, 2024 and took effect on October 21, 2024, and it lets the FTC seek civil penalties against knowing violators.
FDA warned on April 16, 2024 that counterfeit Botox had been found in multiple states and given for cosmetic purposes, causing adverse events including hospitalizations. CDC's advice: get botulinum toxin injections only if the product comes from an authorized supplier and the provider is licensed and trained — "If in doubt, don't get the injection."
Find a chain location near you
Every chain location count in this article came from that chain's own website, and each chain's site is also the live source for its current locations — location pages change more often than articles do.
Before you book at any location, chain or independent, two checks are worth the time: confirm the specific location is operating as usual (Ideal Image's unconfirmed status above shows why), and ask who will perform your treatment — FDA's advice to consumers is to confirm the product comes from an authorized source and to ask whether the provider is licensed and trained to give it.
Frequently asked questions
What is the biggest med spa chain in the US?
By the counts chains publish about themselves, Milan Laser posts the highest one we verified: over 400 locations nationwide (its own site, October 2026).
Restore Hyper Wellness says 210+ studios and LaserAway says over 200 locations.
Each figure is that chain's own "more than" claim, so the order among them is approximate, and Ideal Image's count could not be confirmed because its website showed only a parking page when we checked on October 16, 2026.
How much do med spa chains charge for Botox?
Chain-published examples, read in October 2026: LaserAway lists Botox at $15 per unit, or $10 per unit in a 300-unit bank, and Juvéderm lip filler at $450 per full syringe.
Milan Laser publishes no package prices online and gives personalized quotes.
VIO's ClubVIO Signature membership costs $99 per month and takes $2 off each toxin unit.
LaserAway and Sev Skin state that prices vary by location, so treat any published menu as a starting point rather than a final price.
Do med spa chains sell memberships?
Several do.
Sev Skin says its laser hair removal memberships start at $15 per month; VIO's ClubVIO Signature tier is $99 per month; and Restore says its memberships carry a 3-month minimum commitment and then auto-renew monthly, with credits on most plans expiring 100 days after issuance.
The FTC's click-to-cancel rule was vacated in July 2025 before taking effect, so no FTC click-to-cancel rule currently governs these memberships; state auto-renewal laws and case-by-case FTC Act enforcement still apply.
Who can legally inject at a med spa chain?
The legal rules are the same as at any provider, because who may inject is set by state law; chains can add stricter staffing policies of their own.
AmSpa says injecting botulinum toxin is the practice of medicine, so it must be done by a physician, an independent-practice nurse practitioner or another licensed professional acting within their scope, under rules that vary by state.
Chains describe their own staffing: LaserAway says RNs, NPs or physician associates perform every treatment, and Sev Skin says NPs, RNs, licensed estheticians and laser specialists treat clients.
Is a chain or an independent med spa the better choice?
We don't rank them, and our research does not establish that one is cheaper or safer than the other.
AmSpa's 2024 survey estimated 10,488 US med spas in 2023, with 81% single-location — but its public summary does not say how individually owned franchise studios were counted.
What you can compare directly: published prices and whether they vary by location, membership terms, who performs the treatments, and the state rules that apply to chains and independents alike.
