Lip flip vs lip filler: which do you need?

Lip flip vs lip filler is a choice between two different injectable ways to change how the lips look. A lip flip uses a few units of botulinum toxin to relax the orbicularis oris, the muscle around the lips, so the upper lip rolls outward; lip filler is an injected gel that adds volume. This guide compares how each works, what each costs, how long results last, and which goals each one fits.

Key takeaways

  • A lip flip relaxes the muscle around the lips so the upper lip rolls out; filler adds volume.
  • The lip flip is off-label; lip augmentation is an FDA-approved filler use in adults over 21.
  • We could not confirm how long a lip flip lasts; filler longevity depends on the product and where it is injected.
  • Toxin carries a boxed warning for distant spread; filler's most concerning risk is injection into a blood vessel.
  • Which fits is a question for a licensed provider who has examined your lips.

What are a lip flip and lip filler?

A lip flip is an injection of botulinum toxin into the orbicularis oris, the muscle that circles the mouth.

A 2025 systematic review of 7 studies reports the procedure typically uses 4 to 6 units, relaxing the muscle so the upper lip everts, or rolls outward, without adding volume.

No US neurotoxin label includes the lips, so the lip flip is off-label: Botox Cosmetic's only approved areas are frown lines, crow's feet, forehead lines and platysma (neck) bands.

Lip filler is a gel implant: FDA regulates dermal fillers as medical device implants, approved in adults over 21 for uses that include lip augmentation — Restylane was the first US filler with a lip indication, in October 2011.

For a deep dive on either, see our Lip flip guide and our lip filler guide.

How they differ: onset, longevity, spread and units

The table below sums up the differences the sources actually support.

Lip flip Lip filler
What it is Botulinum toxin, a prescription drug A gel implant, regulated by FDA as a medical device
FDA status Off-label for the lips — no US toxin label includes them Approved for lip augmentation in adults over 21
Amount used 4 to 6 units in studies — no US label includes the lips, so no label dose exists for them Varies; syringes are not all one size — Revanesse Versa, Versa+ and Lips+ syringes are approved in 1.0 mL and 1.2 mL sizes
Onset A 2026 study of 17 women measured the change in upper lip height at day 15 after a 4-unit dose No onset figure in the sources we reviewed
Longevity None we could verify; the effect is temporary Depends on the material and where it is injected; absorbable fillers may need repeat treatment
Serious risks Boxed warning for distant spread of toxin effect Unintended injection into a blood vessel — tissue death, vision problems including blindness, or stroke
Common reactions Difficulty whistling or drinking through a straw, resolving within weeks Kysse lip trial: swelling 90.2%, tenderness 87.4%, bruising 85.8%, most under 2 weeks

These rows mix evidence types — a drug label, device approvals and small trials — so read them as two separate dossiers, not a head-to-head trial.

FDA separately lists less common filler risks — nodules, infection, allergic reaction, necrosis — and rare reported ones including anaphylaxis, filler migration, blindness, stroke and death.

Two different serious-risk profiles

Every US botulinum toxin label carries a boxed warning for distant spread of toxin effect: symptoms including generalized weakness, double vision, drooping eyelids, and swallowing or breathing difficulty, reported hours to weeks after injection — swallowing and breathing problems can be life threatening and there have been reports of death. FDA calls unintentional filler injection into a blood vessel — tissue death, vision problems including blindness, or stroke — its most concerning filler risk.

Lip flip vs lip filler cost comparison

The two are priced on different meters.

Toxins are typically priced per unit or per treatment area, so a treatment's price is roughly its units times the per-unit price — for scale, the labeled Botox Cosmetic frown-line dose is 20 units, while lip flip studies report 4 to 6 units.

Filler prices in our sources are quoted per procedure or per syringe.

ASPS's filler cost page lists an average of $743 for lip augmentation with dermal filler, per its latest statistics — average fees reported by ASPS member plastic surgeons, not med spa prices, with no data year stated.

Chain prices are each chain's own: LaserAway's site (read October 16, 2026) listed Botox at $15 per unit, or $10 per unit in a 300-unit bank, and Juvéderm lip filler at $450 per full syringe.

We could not confirm a current national average fee for toxin injections — the ASPS 2025 statistics report omits minimally invasive fees — and these figures come from different kinds of sources, so they are not like-for-like.

Brand matters: the Botox Cosmetic label says its units cannot be compared to or converted into units of any other botulinum toxin product, so per-unit prices only make sense within a brand — see our guide to all neurotoxin brands.

Which is better for which area or person?

The mechanical difference is the heart of the decision: a lip flip changes the upper lip's position by relaxing a muscle; filler changes the lips' size by adding volume.

In the 2026 study of 17 women, a 4-unit flip raised upper lip height modestly at 15 days with no change in lip volume.

The goals point in different directions.

Someone whose goal is added volume is describing filler's approved use — lip augmentation; someone whose goal is a subtle outward roll of the upper lip, without added volume, is describing the effect the lip flip studies measured.

Lip flip

  • Relaxes the orbicularis oris muscle
  • Rolls the upper lip outward
  • Changes position, not size
  • Off-label use of botulinum toxin

Lip filler

  • Gel implanted in the lip
  • Adds volume
  • Changes size
  • FDA-approved for lip augmentation

We make no recommendation between them: which fits is a question for a licensed provider who has examined your lips.

Can you switch between them?

Both effects are temporary.

The Botox Cosmetic label explains that the toxin's muscle effect slowly reverses as the nerve endings reconnect, and FDA says absorbable fillers are naturally absorbed over time and may need repeat treatment.

We found no study comparing starting with a lip flip versus starting with filler, so we cannot tell you an order.

One caution does exist for switching toxin brands: the Botox Cosmetic label says the effect of giving different botulinum toxin products at the same time or within several months of each other is unknown, and that excessive weakness may be worsened by giving another toxin before a previous one has worn off.

Find med spas offering a lip flip or lip filler near you

This guide does not list individual spas.

Both treatments are injections, and for the toxin side AmSpa, a med spa trade association, says injecting botulinum toxin is the practice of medicine because it is a prescription drug, so it must be done by a physician, an independent-practice NP, or another licensed professional acting within their scope — under rules that vary by state.

For toxin, CDC advises getting injections only from a licensed, trained provider using product from an authorized source.

For filler, FDA says the injection should be considered a medical procedure and advises against buying fillers online, where products could be counterfeit or unapproved.

Before you book anywhere, ask who will do the injecting, what product they plan to use, and where they source it.

What a lip flip does vs lip filler

Botulinum toxin blocks acetylcholine, the messenger nerve endings release to tell a muscle to contract; the Botox Cosmetic label describes the result as a partial, localized reduction in muscle activity that slowly reverses as the nerve endings reconnect.

A lip flip aims that relaxation at the orbicularis oris, so the upper lip everts instead of being held inward — the 2025 review describes upper-lip eversion and improved contour, with no volume added.

Filler adds volume directly: the gel is implanted in the lip.

The 2026 study framed the contrast exactly that way — the flip enhances lip projection through selective neuromodulation, not volumetric augmentation.

Results and how long each lasts

For the lip flip, we could not confirm how long it lasts.

No US label covers the lips, and we found no high-quality trial source for a duration — the 2025 review's abstract does not state one.

The 6-to-12-week numbers online come from clinic and consumer websites, not from the label or the review's abstract, so we will not print them as fact.

The one hedge our research allows: results are temporary and generally shorter than in the upper face because a small dose is used — an expectation, not a measured figure.

For scale, the Botox Cosmetic label gives approximately 3 to 4 months for frown lines — a labeled area, not the lips.

Filler longevity depends on the material and where it is injected, FDA says, and absorbable fillers are temporary and may need repeat treatment.

For two lip-approved products: Allergan says Volbella XC lip results can last up to 1 year with optimal treatment, and in Restylane Kysse's pivotal trial 60% of subjects were still responders at week 48 — one maker's claim and one trial, not a promise for every product or person.

Can you combine them?

If you are wondering about doing both at once — a lip flip with filler — we found no study that tests the combination on lips, so we cannot describe how it performs.

The nearest sourced analog is about wrinkles, not lips: Cleveland Clinic describes Botox as treating lines that appear when a muscle contracts, and says people whose lines are deep even at rest may benefit from adding a filler.

Combining would also mean two injectable procedures in one visit, each with its own risk profile — the toxin's boxed warning and the filler's vascular risk, both described in full under "How they differ" above.

Both risks would be in one visit

Botulinum toxin labels carry a boxed warning for distant spread of toxin effect: swallowing and breathing problems can be life threatening and there have been reports of death. FDA's most concerning filler risk is unintentional injection into a blood vessel, which can cause tissue death, vision problems including blindness, or stroke.

Which is better for thin lips or a gummy smile?

Thin lips come down to the volume question: a lip flip adds no volume — the 2026 study measured a modest lip-height increase with no volume change at 4 units — while adding volume is what fillers are approved to do in adults over 21.

A gummy smile is a different treatment from either: the studied toxin target is the muscles that lift the lip (levator labii superioris alaeque nasi, levator labii superioris, zygomaticus minor), not the orbicularis oris.

A 2024 review of 13 controlled trials found maximum effect at 2 to 4 weeks, lasting up to 12 to 24 weeks; a 2023 meta-analysis reported 1.25 to 7.5 units per side, with gum exposure reduced by up to 6 mm at 12 weeks — study figures, not label doses.

Gummy smile toxin is off-label like the flip: Cleveland Clinic lists "lip lengthening to correct gummy smile" among the off-label Botox uses it offers, and none of these areas is on any current US label.

We could not confirm filler studies for gummy smile in our sources.

Frequently asked questions

Is a lip flip cheaper than lip filler?

There is no head-to-head price we can cite.

A lip flip uses a small number of units — 4 to 6 in the studies — and toxins are priced per unit or per area, so cost scales with units.

Filler prices in our sources are per procedure or per syringe: ASPS lists an average of $743 for lip augmentation with filler (member plastic surgeons' fees, no data year given), and LaserAway's site listed Juvéderm lips at $450 per syringe and Botox at $15 per unit as of October 16, 2026.

How long does a lip flip last?

We could not confirm a verified figure.

No US toxin label covers the lips, and we found no high-quality trial source for how long the effect lasts — the 2025 systematic review's abstract does not state a duration.

The 6-to-12-week figures you see online come from clinic and consumer websites, not from the label or the review's abstract.

The hedge our research allows is that the effect is temporary and generally shorter than upper-face toxin results, because the dose is small — an expectation, not a measured figure.

For scale, the Botox label's approximately 3 to 4 months is for frown lines, a labeled area.

Does a lip flip make your lips bigger?

No — bigger is not what it does.

A lip flip relaxes the orbicularis oris so the upper lip rolls outward, which changes the lip's position, not its size.

In a 2026 study of 17 women, upper lip height increased modestly at 15 days but lip volume did not change.

Adding volume is what lip filler does, and lip augmentation is an FDA-approved filler use in adults over 21.

Which is better for a gummy smile, a lip flip or filler?

The evidence we hold on gummy smile is about botulinum toxin into the lip-elevator muscles — a different target from the lip flip's orbicularis oris.

A 2024 systematic review of 13 controlled trials found maximum effect at 2 to 4 weeks, lasting up to 12 to 24 weeks, and a 2023 meta-analysis reported 1.25 to 7.5 units per side with up to 6 mm less gum exposure at 12 weeks.

It is off-label — no US label includes it — and we could not confirm any filler studies for gummy smile in our sources.
