Med Spa Rules in Iowa

What Iowa law and its licensing boards say about who may do med spa treatments, each with its source. This is a plain-English summary, not legal advice.

  1. The Iowa Board of Medicine adopted a medical spa rule, Iowa Administrative Code 653-13.6, which sets standards for medical directors at medical spas and took effect May 21, 2025. [1]
  2. Iowa's rule lists Botox and collagen injections, other injectables, ablative and non-superficial laser, IPL, radiofrequency and ultrasound therapy, microneedling, fat freezing and tattoo removal as medical aesthetic services, but hair removal is not on the list. [1]
  3. In Iowa, a medical spa is any business, however it is set up, that is advertised or run to provide medical aesthetic services. [1]
  4. A dermatology practice wholly owned by Iowa-licensed physicians, with an owner practicing at each location, is not covered by Iowa's medical spa definition. [1]
  5. An Iowa medical spa's physician medical director must make sure staff get direct, in-person, on-site supervision from the medical director or another qualified supervisor at least four hours each week, and must review services on site weekly, including at least 10% of patient charts. [1]
  6. The medical director must stay within 60 miles of the medical spa at all times and be available in person or electronically at all times. [1]
  7. The medical director and all practitioners at an Iowa medical spa must complete at least 20 hours of continuing education in medical aesthetic services each year. [1]
  8. At an Iowa medical spa, a nonphysician, such as someone licensed or certified by an Iowa health or skin care board, may perform Botox, fillers and other medical aesthetic services only when delegated by a supervising medical director. [1]
  9. Iowa recognizes a qualified laser technician, licensed or unlicensed, who has completed at least 120 hours of accredited laser training, including 40 classroom hours and 80 clinical hours. [1]
  10. A qualified laser technician may use lasers at a medical spa only for delegated services under physician supervision, and an unlicensed technician may not perform any other medical aesthetic services. [1]
  11. Iowa cosmetology board rules say its licensees may use lasers and IPL devices only on the surface layers of the skin, except for hair removal. [2]
  12. Under those cosmetology board rules, laser or IPL hair removal on a minor requires a parent or guardian and general physician supervision. [2]
  13. Iowa cosmetology board rules say licensed estheticians may perform medical aesthetic services in a medical spa only under a medical director's delegation and supervision, and the board does not license medical aestheticians. [2]
  14. Iowa nurse practitioners (ARNPs) may prescribe, administer and dispense prescription drugs, including controlled substances, within their role and population foci, and Iowa rules let them practice to the full extent of their license, education and experience. [3]
  15. An Iowa medical spa must name its medical director in all advertising, websites and signage, and must visibly display and give patients in writing the identity and licensure of the medical director and all practitioners. [1]
