Lip flip: what it is, cost, results and how long it lasts

A lip flip is an off-label botulinum toxin injection that relaxes the orbicularis oris, the muscle that circles the mouth, so the upper lip rolls slightly outward. It does not add volume; adding volume is what lip filler does. No US neurotoxin label includes the lips, so the treatment is off-label. Published studies describe small doses of 4 to 6 units, subtle changes and mild, temporary side effects.

Key takeaways

  • A lip flip is off-label: no US neurotoxin label includes the lips or the orbicularis oris.
  • It everts the upper lip by relaxing a muscle and adds no volume; adding volume is what lip filler does.
  • Studies describe 4 to 6 units and mild, temporary side effects such as difficulty whistling or using a straw.
  • We found no label or controlled trial that establishes how long a lip flip lasts.
  • We could not confirm a reliable national average price, so ask whether a practice charges per unit or per area.

What a lip flip is (and is not)

A lip flip is a botulinum toxin injection into the orbicularis oris, the muscle that circles the mouth.

The toxin partially relaxes that muscle, so the upper lip everts — rolls slightly outward and upward — changing the lip's resting position and contour.

Two things a lip flip is not.

It is not a volumizing treatment: a 2026 study of 17 women found the flip increased upper lip height modestly with no change in lip volume, and a 2025 systematic review describes upper-lip eversion without added volume.

It is also not an approved use. Botox Cosmetic (onabotulinumtoxinA) is FDA-approved in adults for exactly four cosmetic areas — frown lines, crow's feet, forehead lines and platysma (vertical neck) bands — and no US neurotoxin label, including Dysport, Xeomin, Jeuveau, Daxxify and Letybo, lists the lips or the orbicularis oris.

Using a toxin on the lips is therefore off-label for every one of these brands.

Searches for a "Botox lip flip" or "lip botox" refer to this same off-label use.

No brand is approved for the lips, and the studies mostly report onabotulinumtoxinA-equivalent doses without standardizing by brand.

Units are also brand-specific: Botox Cosmetic's label states its units cannot be compared to or converted into units of any other botulinum toxin product, and Dysport's label says the same about its own.

Cleveland Clinic, which offers several off-label toxin treatments, lists lip-related uses such as treating lip lines and lip lengthening for gummy smile among its off-label offerings.

The lip flip belongs to the same family — a studied, but unapproved, use of an approved drug.

What "off-label" means

Off-label means a licensed provider uses an FDA-approved drug for a purpose its label does not list. Here it also means no label sets a dose for this use — the numbers on this page come from the published research, not from a label.

Who is a good candidate

Because the flip changes lip position rather than volume, the honest starting point is the change you want.

The studies measure a modest eversion of the upper lip — in the 2026 study, a small increase in upper lip height at 15 days, with lip volume unchanged.

Someone hoping for noticeably fuller lips is describing a filler result, not a flip result.

The evidence base is small, and worth knowing before you decide.

The 2025 systematic review included 7 studies out of 48 screened, and the 2026 prospective study followed 17 women.

Both report mild effects; neither is a large trial.

The Botox Cosmetic label's exclusions apply to any use of that product, including an off-label one.

It is contraindicated in people with known hypersensitivity to any botulinum toxin product or any ingredient in the formulation, and when there is an infection at the planned injection site.

The label also warns that people with neuromuscular disorders — myasthenia gravis, Lambert-Eaton syndrome, ALS — may be at increased risk of clinically significant effects, and advises caution in people with pre-existing cardiovascular disease or swallowing or breathing problems.

Pregnancy and breastfeeding are a separate gap: there are no adequate human data on Botox Cosmetic in pregnancy and no data on whether it passes into breast milk, so safety in either is not established.

Function is the other consideration the studies flag.

Relaxing the orbicularis oris made whistling and drinking through a straw harder for some subjects.

People whose work or hobbies depend on precise lip control have a specific question to raise with a provider.

None of this decides the answer for any one person.

A licensed provider who examines your lip anatomy and how it moves can tell you whether a flip, a filler or neither matches the change you are describing.

How much it costs

Botulinum toxin is typically priced per unit or per treatment area.

A lip flip uses few units by toxin standards: the 2025 systematic review reports a typical dose of 4 to 6 units into the orbicularis oris.

Under per-unit pricing, the bill is those units times the practice's price per unit; under per-area pricing, the flip is priced as its own area regardless of the unit count.

Per-unit pricing

  • The bill is the units used times the practice's price per unit
  • Studies report a typical dose of 4 to 6 units

Per-area pricing

  • The flip is priced as its own area
  • The price does not depend on the unit count

Two cautions apply when you compare quotes.

First, there is no label dose for the lips, so the unit count is the provider's judgment rather than a labeled recipe.

Second, units are not comparable between brands — Botox Cosmetic's label states its units cannot be converted into another toxin's units, and Dysport's label says the same — so a quote written in one brand's units cannot be translated into another brand's.

On the national picture, we will be direct: we could not confirm a reliable average price for a lip flip.

We found no current ASPS national average fee for neuromodulator injections — the ASPS 2025 statistics report omits fees for minimally invasive procedures — and the lip-flip studies we reviewed report no costs.

Any dollar figure you see quoted as the national lip flip price is not backed by a source we could find.

When you ask a practice for a price, the useful questions are structural: whether they charge per unit or per area, which toxin they would use, how many units they plan for your anatomy, and whether a follow-up visit is included.

Each practice sets its own prices, and the only quote that counts is the one for the product and dose planned for you.

The pricing structures are broken down further in lip flip cost.

Results timeline and how long it lasts

The controlled evidence on the flip's own timeline is thin.

In the 2026 study, researchers measured a modest increase in upper lip height at 15 days in 17 women given 4 units of onabotulinumtoxinA.

That is when a change was measurable — not necessarily when it first appeared.

What the change looks like in photos is covered in lip flip before and after.

How long the result lasts is a question the research does not answer.

We found no FDA label or high-quality controlled trial reporting a duration for the lip flip; the "6 to 12 weeks" and "2 to 3 months" figures that circulate online come from clinic and consumer websites, not from the label or a controlled study.

Treat any specific week count you are quoted as an estimate from practice experience, not a tested figure.

Mechanically, the effect is temporary by design.

The toxin partially blocks the muscle's nerve signal, and the nerve-muscle connection slowly regrows, reversing the block.

Because the flip uses a small dose, providers reasonably expect a shorter effect than upper-face treatment — that is a reasoned expectation, not a measured figure, and we found no trial that has tested it.

One labeled area does have a measured duration for contrast: the Botox Cosmetic label gives 3 to 4 months for frown lines, an area and dose FDA has reviewed.

See how long Botox lasts for the area-by-area data.

The nearest controlled evidence around the mouth is a different off-label use — gummy-smile treatment — where a 2024 systematic review of 13 controlled trials found maximum effect at 2 to 4 weeks and effects lasting up to 12 to 24 weeks.

That is not a lip flip duration; it is a different treatment acting on different muscles.

It is the closest controlled data we found, and it is not for the flip.

Risks and what can go wrong

The lip-flip studies report mild effects.

The 2025 systematic review describes mild, transient side effects — difficulty whistling or drinking through a straw — resolving within weeks.

The 2026 study of 17 women reported temporary numbness and functional limitation, rated mild, that resolved within 30 days.

30 daysMild side effects in the 2026 study resolved withinTemporary numbness and functional limitation; 17 women

The Botox Cosmetic label lists reactions that may come with the injection itself: localized pain, swelling, redness, tenderness and bruising.

It says adverse reactions generally occur within the first week and, while often transient, may last several months or longer.

The labels list these reactions themselves — injection pain, bruising, swelling, headache and eyelid drooping among them — so they are part of the treatment's own documents rather than scare stories.

The flip adds a functional risk of its own: the same muscle relaxation that everts the lip can interfere with what the lips do.

Difficulty whistling or using a straw is the review's own example, and the study's "functional limitation" fits the same pattern.

Ask the provider who treats you what should settle in the first weeks — in the studies, the reported effects resolved within weeks to 30 days.

The toxin boxed warning applies to every US brand

All botulinum toxin products carry an FDA boxed warning for distant spread of toxin effect: generalized weakness, double vision, drooping eyelids, or swallowing or breathing difficulty, reported hours to weeks after injection. The label says swallowing and breathing problems can be life threatening, and there have been reports of death. The Medication Guide says to seek immediate medical help for swallowing, speaking or breathing problems.

Lip flip vs lip filler

The difference is mechanical.

A lip flip everts the upper lip by relaxing a muscle and adds no volume; lip filler adds volume.

The regulatory picture is the mirror image of the mechanism.

Filler is regulated by FDA as a medical device implant — not a drug — with specific allowed uses in adults over 21 that include augmentation of the lips, while the flip is off-label for every US toxin brand.

The treatment that adds volume is the one with FDA-listed lip uses; the treatment that changes position has none.

Duration is described differently too.

FDA says how long a filler lasts depends on the filler material and where it is injected, and that absorbable fillers are temporary and may need repeat treatment.

For the flip, we found no established duration.

Lip filler

  • Regulated by FDA as a medical device implant

Which one fits is a question about your anatomy and your goals, and it has no general answer we can responsibly give.

A licensed provider can examine your lips and show you what each option would and would not change.

Filler's most concerning risk is vascular

FDA warns that the most concerning risk with dermal fillers is unintentional injection into a blood vessel, which can cause tissue death, vision problems including blindness, or stroke. That vascular risk is described for fillers; the serious risk listed on toxin labels is the boxed warning for spread of toxin effect above.

Find a lip flip near you

Because the flip is off-label, the provider matters more than the price.

CDC advises consumers to get botulinum toxin injections only from a licensed, trained provider using product from an authorized source, and treats the procedure as carrying risk even when FDA-approved product is given correctly.

Product sourcing has a recent failure mode worth knowing.

On April 16, 2024, FDA warned that counterfeit Botox had been found in multiple states and injected for cosmetic purposes, with adverse events including hospitalizations.

FDA said the incidents involved licensed and unlicensed individuals and non-medical or unlicensed settings, with product apparently bought from unlicensed sources.

The Botox Cosmetic label says genuine vials and cartons show U.S. License number 1889, and instructs that product missing that number not be used.

Questions worth asking at a consultation: how often the injector does a lip flip specifically, which toxin they use and why, how many units they plan for your lip anatomy, and what their plan is if you report an effect afterward.

CDC also notes that your state may offer a look-up tool to check whether a provider or setting holds the appropriate license.

Who may inject botulinum toxin, and under what supervision, varies by state: AmSpa, a medical spa trade association, says injecting it is the practice of medicine because a prescription drug is used, so it must be done by a physician, an independent-practice nurse practitioner or another licensed professional acting within their scope — under rules that vary by state.

CDC's advice to check licensing through your state covers that difference.

Frequently asked questions

Does a lip flip hurt?

It is an injection, and the Botox Cosmetic label lists localized pain, swelling, redness, tenderness and bruising among reactions that can come with the injection; it says adverse reactions generally occur within the first week.

In the 2026 study of 17 women, reported effects such as temporary numbness were mild and resolved within 30 days.

The labels list these reactions themselves, so they are part of the treatment's own documents.

Is a lip flip FDA-approved?

No. No US neurotoxin label — Botox Cosmetic, Dysport, Xeomin, Jeuveau, Daxxify or Letybo — includes the lips or the orbicularis oris among its approved areas.

Botox Cosmetic's four approved cosmetic areas are frown lines, crow's feet, forehead lines and platysma bands.

The lip flip is an off-label use: it is on no current US label.

How many units does a lip flip use?

A 2025 systematic review of 7 studies reported a typical lip flip dose of 4 to 6 units of botulinum toxin into the orbicularis oris.

There is no label dose for the lips, so the amount is the provider's judgment.

Units are also brand-specific: Botox Cosmetic's label says its units cannot be converted into another brand's, and Dysport's says the same, so a quote in one brand's units is not translatable to another.

How long does a lip flip last?

We found no FDA label or high-quality controlled trial reporting a duration for the lip flip; the "6 to 12 weeks" figures you see online come from clinic and consumer websites, not controlled research.

The effect is temporary because the nerve-muscle connection slowly regrows.

Because a small dose is used, a shorter effect than in the upper face is a reasonable expectation, but it is not a measured figure and we found no trial that has tested it.

Lip flip vs lip filler: which is better?

They do different things, so "better" depends on the change you want.

The flip everts the upper lip by relaxing a muscle and adds no volume; filler adds volume.

Filler lip augmentation is an FDA-regulated medical device use for adults over 21, while the flip is off-label for every US toxin brand.

Their serious risk profiles differ too: FDA's most concerning filler risk is unintentional injection into a blood vessel.

A licensed provider can assess which matches your anatomy.
