Key takeaways
- Hollow cheeks are mainly a volume problem, and six US fillers carry an FDA cheek/midface volume indication as of 2026-10-20.
- Sculptra is not one of them: its cheek indication is fine lines and wrinkles, so filling hollow cheeks with it is off-label.
- No label or trial we checked sets a standard syringe count. The label maximum is a ceiling, not a target.
- Allergan's “up to 2 years” for Voluma XC cheeks is an upper bound. In the head-to-head trial, 62.8% (Contour) and 65.1% (Voluma) of subjects were still responders at week 48.
What causes hollow cheeks?
Hollow cheeks are a volume problem first.
The FDA approvals describe it in their own words: Juvéderm Voluma XC is approved for cheek augmentation to correct “age-related volume deficit in the midface,” and Restylane Contour for correction of “midface contour deficiencies” — label language for a midface that has lost fullness it once had.
In the sources we checked, why cheeks hollow is mostly manufacturer framing, so attribute it: Revance says its RHA Dynamic Volume filler addresses midface (cheek) volume loss due to factors such as aging, weight loss and sun damage, and Merz says rapid weight loss — especially from GLP-1 medications — can lead to visible changes in the face, such as hollowness or sagging.
One of the first quantified looks at the weight-loss question is a small 2025 imaging study: 20 patients taking GLP-1 medications (mean weight loss 11.0 kg) showed a median 9.0% decrease in total midface volume, mostly from the superficial fat pads.
It is one retrospective study of 20 people, not a measurement of your face, and “Ozempic face” is not a medical diagnosis.
Hollowing is also one face of the broader process of facial volume loss.
Whether your own midface is a volume question at all — and whether filler matches it — is exactly what a licensed provider's examination is for.
When volume is the cause, filler is the treatment class built for it, which is what the rest of this page covers.
HA filler vs Sculptra for hollow cheeks
For volume itself, the products with FDA approvals are specific and few.
As of 2026-10-20, six US fillers carry a cheek/midface indication: Juvéderm Voluma XC (2013), Restylane Lyft (2015), Restylane Contour (2021), RHA 4 and RHA Dynamic Volume (2025) and saypha ChiQ (2026).
The Juvéderm and Restylane products among them are crosslinked hyaluronic acid gels with lidocaine — the “HA” of this section's heading.
Allergan's Voluma XC label specifies deep (subcutaneous and/or supraperiosteal) injection — gel placed in the deeper midface layers, not within the skin itself.
Sculptra, though, is not one of them — and that is the mix-up to catch.
It is poly-L-lactic acid, not hyaluronic acid, and rather than a gel placed for volume, Galderma says it works by stimulating the body's own collagen.
Its FDA indications are shallow to deep nasolabial fold contour deficiencies and other facial wrinkles, fine lines and wrinkles in the cheek region, and facial fat loss in people with HIV.
Cheek volume is not among them, so using Sculptra to fill hollow cheeks is an off-label use — it is not on any current US label for the product.
The two also behave differently after injection.
Allergan says Juvéderm results are visible immediately after treatment.
Sculptra is a series: in its trials, patients were treated to optimal correction at 3- or 4-week intervals with a maximum of four sessions, and the fullness right after a session is injection swelling, which resolves within hours to a few days — the original depression reappears first, and correction then develops gradually over weeks.
HA fillers (e.g. Voluma XC, Contour)
- FDA cheek/midface volume indication
- Crosslinked hyaluronic acid gel with lidocaine
- Allergan says Juvéderm results are visible immediately
Sculptra
- Cheek indication is fine lines and wrinkles, not volume
- Poly-L-lactic acid that stimulates your own collagen
- A series of sessions; correction develops gradually over weeks
Unintentional injection into a blood vessel is the risk FDA calls most concerning with dermal fillers: the chance is low, but it can block blood supply and cause tissue death (necrosis), vision abnormalities including blindness, or stroke. Juvéderm's Important Safety Information says such events can be serious and possibly permanent, and that most of them are irreversible.
How much filler do hollow cheeks need?
There is no syringe count we can hand you — we could not confirm a standard number for hollow cheeks in any label, trial or professional-body document we checked.
What exists are ceilings and medians.
The Restylane Contour label caps each treatment session at 6 mL in the cheeks and midface; that is the most the label allows per session, not an amount you should expect to receive.
The trial medians sit below it.
In Restylane Contour's cheek trial, the median total volume injected for cheek augmentation was 3.80 mL for the initial and touch-up treatments combined — both cheeks together.
Those are medians from one product's study — not a dose, and not a prediction for any individual face.
Money follows the amount: Allergan publishes no price on its Juvéderm FAQ and says only that pricing varies by the product, the amount used and the provider.
What cheek filler should cost, and the national figures we can cite, is covered in full on our cheek filler guide.

How long does filler last in hollow cheeks?
Allergan says Juvéderm results are visible immediately after treatment — and the duration claim that follows is a careful one.
Allergan states Voluma XC results in the cheeks last “up to 2 years with optimal treatment.” That is an upper bound, tied by Allergan's own wording to optimal treatment; it is not a typical result and not a promise.
The trial record is more sober.
In the head-to-head cheek study — Restylane Contour against Juvéderm Voluma XC, sponsored by Contour's maker Galderma — blinded evaluators still rated 62.8% of Contour subjects and 65.1% of Voluma subjects responders at week 48, close to the one-year mark.
Responder rates decline over the year rather than switching off on a day.
Blinded-evaluator responder rates, head-to-head cheek trial (Galderma)
| Visit | Restylane Contour | Juvéderm Voluma XC |
|---|---|---|
| Week 12 | 91.2% | 87.7% |
| Week 24 | 88.5% | 83.1% |
| Week 36 | 72.1% | 83.6% |
| Week 48 | 62.8% | 65.1% |
Sculptra's clock runs differently: Galderma says results appear as early as 1 month after treatment and last up to 2 years — again a maker's “up to,” from the company marketing the product.
And when the Contour trial's subjects were re-treated at week 48, the median volume was 1.95 mL, against 3.80 mL for the initial plus touch-up — a top-up, not a restart.
None of these numbers predicts your result: each comes from one product's trial or one maker's marketing page, and a maker's “up to” is an upper bound, not an individual forecast.
A licensed provider who has examined your midface can tell you what to expect in your case.
Frequently asked questions
How many syringes of filler do you need for hollow cheeks?
There is no established number we can cite.
The Restylane Contour label caps each cheek/midface session at 6 mL, and in that product's cheek trial the median volume for both cheeks was 3.80 mL including the touch-up — a study median, not a target.
The amount for your face is set at the consultation — ask for the planned product, the volume and the total price in writing.
Is Sculptra FDA-approved for hollow cheeks?
No. Sculptra's FDA indications are shallow to deep nasolabial fold contour deficiencies and other facial wrinkles, fine lines and wrinkles in the cheek region, and facial fat loss in people with HIV.
Cheek volume is not among them, so filling hollow cheeks with Sculptra is an off-label use.
It is also a series — up to four sessions at 3- or 4-week intervals in its trials — rather than a one-visit treatment.
Does cheek filler work right away?
Allergan says Juvéderm results are visible immediately after treatment.
Sculptra is slower: the fullness right after a session is injection swelling, which resolves within hours to a few days, correction then develops gradually over weeks, and Galderma says results appear as early as 1 month and last up to 2 years.
Can filler fix cheeks hollowed by weight loss?
The manufacturers frame it that way: Revance names weight loss among the factors its RHA Dynamic Volume filler addresses, and Merz says rapid weight loss — especially from GLP-1 medications — can lead to visible changes in the face, such as hollowness or sagging.
One 2025 imaging study of 20 GLP-1 patients measured a median 9.0% decrease in total midface volume.
No filler carries an FDA indication specifically for weight-loss-related volume loss, so whether filler fits your case is a judgment for a licensed provider.
