Revitalize Health & Body is the only locally owned and operated Health and Wellness Clinic in Callahan, FL. We provide personalized, holistic patient care and experiences for all our service lines which include: Direct Primary Care, Weight loss, Hormone Replacement Therapy (for men & women), Peptide Therapy, Aesthetics (Botox/Xeomin) and IV Hydration Therapy. We also offer sports and school physicals. We are happy to serve our local Nassau County community as well as the state of Florida & Georgia with telehealth services.
