How Much Does TRT Cost?

How much does testosterone replacement therapy cost? Our research found no single confirmed figure. TRT is an ongoing prescription - medication, lab work and provider visits - and programs can bundle those pieces differently: flat monthly memberships, itemized billing, or telehealth subscriptions. Because no national reference price from a named survey or professional body turned up in that research, this page explains how the pricing is built and what to ask before you commit to one.

Key takeaways

  • Our research found no national reference price for TRT from a named survey or professional body.
  • A TRT bill combines medication, laboratory work and provider visits, and programs bundle them differently.
  • Lab charges recur: labels call for periodic hemoglobin and hematocrit checks during long-term therapy.
  • Telehealth flexibilities that waive the in-person visit run through December 31, 2026.
  • Ask every program for the all-in monthly price in writing.

What does TRT cost per month?

TRT is recurring care rather than a one-time procedure, and the charges recur with it.

A month on treatment involves charges for the medication itself, laboratory testing, and time with the prescriber, and programs can bundle those pieces differently: a flat monthly membership, itemized billing, or a subscription with parts pushed through insurance.

Two quoted prices for TRT may not be priced against the same list.

What the treatment involves clinically - the forms, the label's testing and monitoring requirements - is covered in our TRT guide.

We could not find a national reference price for TRT from a named survey or professional body in our research, so no monthly average can be honestly quoted here.

What the research does show is why the medication line varies from patient to patient.

FDA states that current FDA-approved testosterone formulations include oral, topical gel, transdermal patch, buccal system and injection.

The testosterone cypionate (Depo-Testosterone) label, for one form, gives 50-400 mg every two to four weeks for hypogonadal men, with the dose adjusted to response and side effects - so the amount one patient uses in a month is not the amount another uses.

The recurring shape of the billing matters as much as any single number.

Testosterone labels call for periodic hemoglobin and hematocrit checks during long-term therapy, so laboratory charges are not just a start-up event - they recur.

A headline monthly price can grow once testing and visits are added back, which is why the written all-in figure is the one to ask for.

Ask for the all-in monthly number

Before signing anything, have a program state in writing what the monthly price covers: which medication and formulation, the laboratory work, provider visits, supplies and shipping, and what happens to the price if any of those pieces change.

What should a TRT price include?

Whatever the payment model, a complete program has to cover the steps the label itself requires.

Before starting treatment, the Depo-Testosterone label says to confirm hypogonadism with morning serum testosterone measured on at least two separate days, both below the normal range - so ask whether a program bills that testing inside an enrollment fee or as a separate charge.

The medication is the other large piece.

FDA lists oral, topical gel, transdermal patch, buccal system and injection among current approved testosterone formulations.

The injection example shows how far the supply math can range: the testosterone cypionate label gives 50 to 400 mg every two to four weeks, with the dose adjusted to response and side effects, so two patients on the same form can use very different amounts in a month.

Then there is ongoing monitoring.

The labels call for periodic hemoglobin and hematocrit checks during long-term therapy to detect polycythemia, and the Depo-Testosterone label says to monitor blood pressure periodically and that testosterone is not recommended with uncontrolled hypertension.

Ask any program how often it schedules that testing and how each piece is billed.

When you compare programs, the monitoring schedule is part of what the price should account for.

Does insurance pay for testosterone replacement therapy?

We could not confirm in our research how individual health plans treat TRT coverage, so that answer has to come from your insurer and the clinic's billing office.

What can be described precisely is the documented starting point a claim would rest on.

FDA-approved testosterone is indicated for replacement therapy in males with conditions associated with deficiency or absence of endogenous testosterone, with hypogonadism confirmed by morning serum testosterone below the normal range on at least two separate days before starting, and FDA says testosterone is approved solely for men who lack or have low testosterone in conjunction with an associated medical condition.

The practical questions are for the billing office.

Ask which pieces - visits, laboratory work, the medication - would be billed to insurance and which are charged as a flat program fee, and ask what you would pay if a claim is denied.

Get both answers in writing before you commit to anything recurring.

Approved for diagnosed deficiency, not aging

FDA says testosterone is approved solely for men who lack or have low testosterone in conjunction with an associated medical condition, and that safety and efficacy for age-related (late-onset) hypogonadism have not been established. A program selling TRT for energy, muscle building or anti-aging is not describing a labeled use.

Telehealth TRT vs a local clinic

Telehealth is where the law reaches into the price.

Testosterone is a Schedule III controlled substance under federal law, listed as an anabolic steroid in 21 CFR 1308.13(f), so prescribing it follows federal rules ordinary prescriptions do not.

Under the Ryan Haight Act, a practitioner normally may prescribe a controlled substance by telemedicine only after at least one in-person medical evaluation.

DEA and HHS's fourth temporary extension keeps the COVID-era telemedicine flexibilities - no in-person visit required - in effect from January 1, 2026 through December 31, 2026.

The end date is the fact to carry into any price comparison.

Online TRT programs are operating under rules that currently expire on December 31, 2026, and we could not confirm in our research what rules will apply after that date, so check the current DEA rules before you sign.

If the flexibilities lapse or are replaced, how the prescription is delivered could change, and with it what a telehealth subscription has to include and charge.

A local clinic that does the evaluation in person is not exposed to the same deadline.

Telehealth program

  • Operates under DEA/HHS flexibilities that waive the in-person visit
  • Those flexibilities currently expire on December 31, 2026
  • What the subscription includes and charges could change if they lapse

Local clinic

  • Does the evaluation in person
  • Not exposed to the same telemedicine deadline
  • Compare it on the same list: testing, medication, monitoring, cancellation

Whichever model you compare, work from the same list rather than the headline fee: whether confirmatory testing is included or extra, which medication and formulation the price assumes, how often monitoring is scheduled, whether supplies and shipping are counted, and what cancellation costs.

TRT prices near you

Because no national reference price for TRT was found in our research - no figure from a named survey or professional body - the dependable prices are the ones you collect locally.

A membership that bundles medication, laboratories and visits is a different product from itemized clinic billing plus a separate pharmacy charge, and ranking those prices against each other without naming what each includes says nothing.

The list below is the script for making quotes comparable.

  • The all-in monthly price in writing, and exactly what it covers
  • Whether the confirmatory testing - morning serum testosterone on two separate days - is billed inside the program or separately
  • Which medication and formulation the price assumes, and what the price becomes if the dose or form changes
  • How often hemoglobin, hematocrit and blood-pressure checks happen, and how each is billed
  • The length of any commitment and what cancellation costs

Vet the provider, not just the number.

The credential, supervision and red-flag questions in our guide to how to choose a med spa are worth asking any provider before you commit to recurring payments for a prescription medication.

Frequently asked questions

Does insurance cover testosterone replacement therapy?

We could not confirm in our research how individual health plans treat TRT, so settle that with your insurer and the clinic's billing office.

What is documented is the label the drug is approved under: FDA-approved testosterone is indicated for replacement therapy in males with conditions associated with deficiency or absence of endogenous testosterone, with hypogonadism confirmed by morning serum testosterone below the normal range on at least two separate days before starting.

Ask which visits and labs would be billed to insurance and what you would pay if a claim is denied.

How much is TRT per month?

There is no national reference price for TRT from a named survey or professional body in our research, so an honest monthly average cannot be quoted.

The bill has moving parts - the medication, which depends on the formulation and the dose you use; periodic laboratory work; and provider visits.

Programs can combine them differently, so get an all-in monthly figure in writing and check exactly what it covers before you sign up.

Can you get TRT without an in-person visit?

Normally, no. Testosterone is a Schedule III controlled substance, and the Ryan Haight Act says a practitioner may prescribe a controlled substance by telemedicine only after at least one in-person medical evaluation.

DEA and HHS's fourth temporary extension keeps the COVID-era flexibilities - no in-person visit required - in effect from January 1, 2026 through December 31, 2026, which is why online TRT programs can operate today.

Confirm the current rules before you sign, because that extension has an end date.

Is TRT FDA-approved for low testosterone from aging?

No. FDA-approved testosterone is indicated for males with conditions associated with deficiency or absence of endogenous testosterone, and FDA says safety and efficacy for age-related (late-onset) hypogonadism have not been established.

Marketing that sells TRT for energy, muscle building or anti-aging is not describing a labeled use.

If you are comparing programs on price, ask each one how the diagnosis will be documented before any prescription is written.
