Key takeaways
- Under-eye filler adds volume to the tear-trough hollow; it does not remove fat-pad bags or treat pigment.
- Four US fillers carry an FDA-approved under-eye indication; any other filler there is off-label.
- In the Eyelight trial, blinded-evaluator responder rates fell from 86.0% at month 6 to 63.5% at month 12.
- Swelling lasted 2 to 7 days for 76.6% of Eyelight trial subjects.
- The Eyelight label warns injecting in people with pigmented dark circles may make discoloration more prominent.
Tear trough filler before and after: the hollow itself
The "before" is the hollow itself: a tear-trough deformity, a depression under the eye that a 2012 review in Aesthetic Surgery Journal describes as a problem separate from under-eye bags — the two can occur together or alone.
The "after" is volume placed into that depression.
As of October 2026, four US fillers carry an FDA-approved indication for it: Juvéderm Volbella XC (approved for infraorbital hollowing in adults over 21 on 2021-05-28), Restylane Eyelight (2023-05-08), Belotero Balance (+) Lidocaine (2023) and RHA Redensity Eye / Eye Lido (2026, adults 22 or older).
Fillers with an FDA-approved under-eye indication
| Product | Under-eye approval | Adults |
|---|---|---|
| Juvéderm Volbella XC | 2021-05-28 | Over 21 |
| Restylane Eyelight | 2023-05-08 | Over 21 |
| Belotero Balance (+) Lidocaine | 2023-09-27 | Over 21 |
| RHA Redensity Eye / Eye Lido | 2026-05-12 | 22 or older |
Any other filler placed under the eyes — Restylane-L, Juvéderm Ultra or Voluma, Radiesse, Sculptra — is an off-label use, with no under-eye indication on that product's US label.
The amounts involved are small: the Restylane Eyelight label caps treatment at up to 1 mL per side per session, and if more is needed it recommends a follow-up session, because the safety of larger amounts per session has not been established.
In the Eyelight trial, the median volume for both hollows together was 1.2 mL at the initial treatment and 1.0 mL at touch-up, with total volume across up to 3 treatments ranging from 0.1 to 6.0 mL.
In a UK single-practice series of 1,452 tear-trough applications in 583 patients, the mean first-visit volume was 0.34 mL per eye.
These are study figures, not an amount any individual needs.
Improvement was then graded by blinded evaluators: 87.4% of treated subjects were responders at month 3, versus 17.7% of untreated controls, with rates of 86.0% at month 6, 77.6% at month 9 and 63.5% at month 12.
A responder showed at least a 1-point improvement on both sides.
Trial rates describe the study group, not what any one person will see.

Our under-eye filler guide covers the approved products, the risks and choosing an injector in depth.
FDA calls unintentional injection into a blood vessel the most concerning dermal filler risk: the chance is low, but it can block blood supply and cause tissue death (necrosis), vision abnormalities including blindness, or stroke. Restylane labels note many reported cases needing intervention involved the glabella, nose and periorbital area.
Under-eye filler before and after dark circles
Dark circles are not one problem.
The 2012 review cited above separates under-eye bags, which come from excess fat, from the tear-trough hollow: surgery (blepharoplasty) removes the fat, while tear-trough treatment adds volume to the depression.
Filler's part is the hollow, not the pigment.
Galderma's site promotes Restylane Eyelight to 'correct dark circles', but Eyelight's approved indication is infraorbital hollowing only, and the label warns that injecting in people with pigmented dark circles may make discoloration more prominent.
So the before-and-after difference depends on what the darkness is.
Where it is the shadow of a hollow, adding volume can soften that shadow.
Where it is pigment, filler is not approved to treat it — and Eyelight's label warns that injecting may make the discoloration more prominent.
Darkness from a hollow
- The shadow of a tear-trough depression
- Adding volume can soften that shadow
- Infraorbital hollowing is the approved indication
Darkness from pigment
- No filler is approved to treat it
- Eyelight's label warns injecting may make discoloration more prominent
Fat-pad bags are a different problem again: filler adds volume rather than removing fat, and the review puts that problem on the surgical side.
Restylane Eyelight is approved to improve infraorbital hollowing. Galderma's site promotes it to 'correct dark circles'. That marketing goes further than the label, which warns that injecting in people with pigmented dark circles may make discoloration more prominent.
Under-eye filler before and after smiling
For the smiling question specifically, we did not find trial data that measures how the under-eye area looks mid-expression: the studies grade the hollow on standardized scales at follow-up visits.
What the studies do record is the path from "treated" to "settled", and that path is what changes how the area looks day to day, smile included.
A 2023 meta-analysis of 31 reports and 2,556 participants found pooled satisfaction of 91.0% after hyaluronic acid filler for tear-trough deformity, with pooled swelling or edema at 19.2% and bruising at 18.4% — averages pooled across the 31 reports, not a forecast for any one person.
In the Eyelight trial diaries after the initial injection, tenderness was the most-reported entry (89.7%), ahead of swelling (85.0%) and bruising (63.1%); swelling lasted 2 to 7 days for 76.6% of subjects and 14 to 28 days for 2.0%.
The median time until subjects felt comfortable returning to social engagement was within 4 hours after needle treatment and within 12 hours after cannula treatment.
Medians from one trial are not a schedule to plan around.
Two warnings matter for how the area looks over time.
The Eyelight label warns that people prone to edema may get excessive swelling from fluid build-up, and that too-superficial injection in thin under-eye skin may leave contour irregularities, palpable lumps or bluish discoloration.
A 2021 review of periocular fillers reports 90% of adverse events around the eye as mild and resolving within a month, but malar edema — delayed swelling unique to the under-eye area — occurred in 11% of patients.
When a hyaluronic acid filler was used, the review notes malar edema can be managed with hyaluronidase.

Male under-eye filler before and after
The four under-eye approvals are written for adults, not for one sex: over 21 for Juvéderm Volbella XC, Restylane Eyelight and Belotero Balance (+) Lidocaine, and 22 or older for RHA Redensity Eye / Eye Lido.
We could not confirm male-specific results: our sources do not give the Eyelight trial's 333 subjects broken down by sex, and we found no tear-trough trial reported separately for men.
The hollow, the amounts, the swelling window and the product limits above are stated for adults generally.
So the before-and-after for a man considering the treatment reads the same on paper.
What deserves the extra attention is the consult: the provider's under-eye experience, which product they plan to use and what its label covers, and photos of their actual under-eye patients.
Frequently asked questions
How soon after under-eye filler will I look normal?
There is no single answer, but the Eyelight trial measured it: the median time until subjects felt comfortable returning to social engagement was within 4 hours after needle treatment and within 12 hours after cannula treatment.
The trial diaries most often recorded tenderness (89.7%), swelling (85.0%) and bruising (63.1%), with swelling lasting 2 to 7 days for 76.6% of subjects.
Trial medians are not a personal schedule — ask your provider what to plan around.
How long does under-eye filler last?
The two sources we have point to about a year.
In the Eyelight trial, 63.5% of treated subjects were still responders at month 12.
Allergan gives Volbella XC an 'up to 1 year' figure under the eyes, with optimal treatment — an upper bound in marketing wording, not a typical result.
Both are product-specific; neither is a countdown for your face.
Is under-eye filler FDA-approved?
Not as a category — approval belongs to specific products.
As of October 2026, four carry an FDA-approved infraorbital (under-eye hollow) indication: Juvéderm Volbella XC, Restylane Eyelight, Belotero Balance (+) Lidocaine and RHA Redensity Eye / Eye Lido.
Any other filler placed under the eyes, such as Restylane-L, Juvéderm Ultra or Voluma, Radiesse or Sculptra, is off-label there.
Ask any provider you consult which product they use and what its label covers.
Will under-eye filler fix dark circles or eye bags?
We found no filler approved to treat dark circles.
Restylane Eyelight is approved for infraorbital hollowing, and its own label warns that injecting in people with pigmented dark circles may make discoloration more prominent.
Where darkness is the shadow of a hollow, adding volume can soften that shadow.
Fat-pad bags are a different problem again: filler adds volume rather than removing fat, and the problem is usually treated surgically (blepharoplasty).
How much filler is used under the eyes?
The study amounts are small.
The Restylane Eyelight label caps treatment at up to 1 mL per side per session; in its trial the median for both hollows together was 1.2 mL initially and 1.0 mL at touch-up.
A UK series of 1,452 tear-trough applications found a mean first-visit volume of 0.34 mL per eye.
These are study figures — the amount for you is a decision to make with your provider.
