Lip Filler Swelling Stages Day by Day

Lip filler swelling usually lasts a few days to about two weeks: Cleveland Clinic puts the usual course at 24 to 48 hours, with up to a week possible, the Restylane Kysse label expects injection-related reactions to resolve on their own within one week, and in the Kysse trial most diary-reported reactions lasted less than 2 weeks. Here is what those windows mean day by day, what each source advises for the first days, and the warning signs FDA says need immediate care.

Key takeaways

  • Cleveland Clinic says lip filler swelling usually goes away after 24 to 48 hours but may take up to a week.
  • In the Restylane Kysse trial, most diary-reported reactions were tolerable and lasted less than 2 weeks.
  • Lumps and firmness are named side effects; in the Kysse trial, masses and nodules typically lasted less than 30 days.
  • Cleveland Clinic typically schedules a follow-up about two weeks after treatment to review the final result.
  • Unusual pain, vision changes, white, gray or blue skin near the injection site, or stroke signs call for immediate medical attention, per FDA.

Day 0 to 1: when swelling starts

Swelling was the most commonly reported reaction in the Restylane Kysse trial's treatment diaries: 90.2% of subjects reported it after initial treatment, alongside tenderness (87.4%) and bruising (85.8%), and most reactions were tolerable and lasted less than 2 weeks.

FDA notes that filler side effects such as swelling and bruising usually appear soon after injection, and that many resolve in days to weeks.

Cleveland Clinic's numbers for the first days: swelling usually goes away after 24 to 48 hours, but it may take up to a week, and injection pain should fade after 12 to 24 hours.

None of these sources names the day swelling peaks; FDA's note that side effects usually appear soon after injection is why this timeline starts on day 0.

What about the amount injected?

Cleveland Clinic states that, on average, 1 mL of filler is injected into the lips, and in the Kysse trial subjects received a total median volume of 0.90 mL in the upper lip and 0.80 mL in the lower lip.

None of the sources we use breaks its swelling timeline down by amount, and we found no study comparing swelling after 0.5 mL versus 1 mL. ("Half a syringe" is a clinic pricing practice rather than a labeled product size, and syringe sizes vary by product.) For what each volume changes about the result itself, see our lip filler before and after guide.

A recovery with symptoms is the expected pattern

The trial behind the label's expectations describes symptoms, not a symptom-free week: 90.2% of Restylane Kysse subjects reported swelling and 87.4% tenderness. The Kysse label itself calls these reactions "anticipated and expected" to resolve within a week, so early swelling is the described course, not on its own a sign that something went wrong.

Days 2 to 3: lumps and asymmetry

Day 2 to 3 sits inside the one-week windows these sources publish.

Cleveland Clinic's usual course ends at 24 to 48 hours, but its range extends to a week, so swelling on day 2 or 3 still fits the timeline it publishes.

Lumps and firmness are named side effects, not surprises. Juvéderm's patient information lists firmness and lumps/bumps among the most common side effects of its Juvéderm fillers and says most resolve within 30 days.

In the Kysse trial, the most common treatment-related adverse events were injection-site mass (10.3% of subjects), bruising (7.6%) and nodules (5.4%); masses and nodules typically lasted less than 30 days, and bruising less than 14 days.

EventShare of subjectsTypical duration
Injection-site mass10.3%Less than 30 days
Bruising7.6%Less than 14 days
Nodules5.4%Less than 30 days

Asymmetry is the one stage word we could not source: we found no study measuring how uneven lips look during the first days, and the trial percentages above describe lumps, not unevenness.

Cleveland Clinic points judgment to a fixed date: it says a follow-up visit is typically scheduled about two weeks after treatment to review the final result.

Unevenness that concerns you before that appointment is a question for the provider who treated you.

Days 4 to 7: settling

This is the window the Restylane Kysse label speaks to directly: injection-related reactions such as bruising, redness, itching, swelling, pain and tenderness are "anticipated and expected to generally resolve spontaneously within one week after injection." Cleveland Clinic's upper bound lands in the same place — swelling may take up to a week.

The trial diary data is why this timeline keeps going past day 7: most reactions ran less than 2 weeks, so the trial's own window runs past the label's one-week expectation.

Allergan publishes its own lip figure for a different product: in its Volbella XC lip study, side effects were mild or moderate and generally lasted 30 days or less.

Swelling that is still visible at day 7 is inside the ranges these sources publish; what none of them does is promise any one person a date.

Week 2: the final result

Two weeks is when Cleveland Clinic schedules judgment: it says a follow-up visit is typically scheduled about two weeks after treatment to review the final result.

The published ranges bracket that visit: most trial diary reactions had cleared in under 2 weeks, while Allergan's Volbella XC figure — side effects generally lasting 30 days or less — and the Kysse trial's masses and nodules, typically under 30 days, describe effects that can still be finishing at the two-week mark.

"Settled" here means the side effects have finished, not that the filler is done: how long lip filler itself lasts is a separate question with its own figures.

Our how long does lip filler last guide covers the duration numbers by brand.

Booking around an event

Cleveland Clinic advises scheduling lip filler at least two weeks before a big event — the same two-week mark as its follow-up review of the final result.

How to reduce swelling

Each source publishes its own instructions, and they are product- or source-specific rather than one universal protocol — your provider's aftercare instructions come first.

Juvéderm's labeling advises avoiding makeup for 12 hours after treatment and minimizing strenuous exercise, extensive sun or heat exposure, and alcoholic beverages in the first 24 hours; the label ties these rules to avoiding temporary redness, swelling and/or itching.

The Restylane Kysse label frames its advice around the swelling itself: it advises minimizing exposure of the treated area to excessive sun, UV lamps and extreme cold until initial swelling and redness have resolved — a duration keyed to your symptoms rather than to a fixed hour count.

Cleveland Clinic's lip-specific list: ice for up to 10 minutes; no lipstick, lip balm or other lip products for at least 24 hours; no touching or puckering the lips, including kissing and sipping from straws; and no exercise, alcohol, smoking or straws for at least 24 hours.

Parts of that list go beyond the labels — the product labels we reviewed do not mention straws or kissing.

What no source on this page does is promise these steps will shrink swelling.

The labels tie their rules to avoiding extra irritation, and none of the three claims a shorter recovery.

When swelling is not normal

The stages above describe the sources' expected course; this one is the boundary of it.

FDA names a set of signs that take you straight to care, and none of them is a stage on the day-by-day list: they involve unusual pain, vision changes, skin color changes near the injection site, or signs of stroke.

Behind that list is the risk FDA calls its most concerning: unintentional injection into a blood vessel — rare, per FDA, but it can cause tissue death (necrosis), vision problems including blindness, and stroke, and the damage may be permanent.

The published literature describes the signs of vascular compromise after hyaluronic acid filler as skin blanching, a lace-like purple pattern (livedo reticularis), slow capillary refill and dusky blue-red discoloration, followed days later by blisters and tissue slough; the true incidence is unknown because of under-reporting.

Swelling can also be part of a serious report rather than a stage.

Post-marketing, the serious adverse events most commonly reported for Restylane Kysse — three or more reports each — included swelling and angioedema alongside ischemia/necrosis, infection and nodules.

FDA also notes that some filler side effects may appear weeks, months or years after injection, so a reaction that starts late sits outside the day-by-day stages on this page.

And if what you are seeing is fullness sitting above the lip's border rather than swelling in the lip itself, our lip filler migration guide covers that separate question.

Warning signs FDA ties to immediate medical attention

FDA tells patients to seek immediate medical attention for unusual pain, vision changes, a white, gray or blue appearance of skin near the injection site, or any signs of a stroke during or shortly after filler injection. These are not wait-and-see items — the wording is immediate.

Frequently asked questions

Do 0.5 ml and 1 ml of lip filler swell differently?

We found no study comparing swelling after 0.5 mL versus 1 mL in the lips, and none of the sources on this page breaks its timeline down by amount.

For scale, Cleveland Clinic states that, on average, 1 mL of filler is injected into the lips, and in the Restylane Kysse trial subjects received a total median volume of 0.90 mL in the upper lip and 0.80 mL in the lower lip.

Ask the provider who treated you what they used and what they expect in your case.

How long should lip filler hurt or feel tender?

Cleveland Clinic says injection pain should fade after 12 to 24 hours.

In the Restylane Kysse trial, 87.4% of subjects reported tenderness, and most diary reactions lasted less than 2 weeks.

Pain that counts as unusual, or that comes with vision changes, white, gray or blue skin near the injection site or stroke signs, is on FDA's immediate-care list rather than the expected-recovery list.

When can I wear lipstick after lip filler?

Cleveland Clinic advises no lipstick, lip balm or any other lip product for at least 24 hours after treatment.

Juvéderm's labeling sets a shorter, general rule of no makeup for 12 hours.

The two published clocks differ, so your provider's instructions take precedence over both.

Does ice help lip filler swelling go down?

Ice is part of Cleveland Clinic's lip filler aftercare, which suggests ice for up to 10 minutes.

What no source on this page claims is a shorter recovery: the Restylane Kysse label advises avoiding excessive sun, UV lamps and extreme cold until initial swelling and redness have resolved, and Juvéderm's labeling ties its first-24-hour rules to avoiding extra redness, swelling and itching.

None of the sources promises to shrink the swelling itself.

When is lip filler settled enough to judge the result?

The judgment point in these sources is Cleveland Clinic's two-week follow-up: it says a follow-up visit is typically scheduled about two weeks after treatment to review the final result.

The published ranges bracket it — the Restylane Kysse label expects reactions to resolve within a week, most trial diary reactions cleared in under 2 weeks, and Allergan says Volbella XC lip side effects generally lasted 30 days or less.

Lumps or unevenness still present at your review are questions for your provider.
