Key takeaways
- We found no FDA-label or controlled-trial figure for how long a lip flip lasts.
- The "6 to 12 weeks" quoted online comes from clinic and consumer sites, not from a label or trial.
- A lip flip is off-label: no US neurotoxin label includes the lips.
- Studies use small doses of 4 to 6 units, and smaller toxin doses wore off sooner in upper-face data.
- Ask your provider which product and how many units they use, and how they arrive at a duration estimate.
The direct answer
We found no tested number.
No FDA-label or high-quality trial source we reviewed states how long a lip flip lasts, and the "6 to 12 weeks" and "2 to 3 months" figures that circulate online come from clinic and consumer websites, not from the label or a controlled study.
There is no label duration to look up, because no US neurotoxin label includes the lips or the orbicularis oris — Botox Cosmetic's approved areas are frown lines, crow's feet, forehead lines and platysma bands, and the other US brands have nothing for the lips either.
The lip flip is an off-label use.
The controlled evidence that does exist is small and early.
A 2025 systematic review, which included 7 studies out of 48 screened, describes doses of 4 to 6 units of botulinum toxin injected into the orbicularis oris, producing upper-lip eversion without adding volume.
One prospective study followed 17 women given 4 units of onabotulinumtoxinA: upper lip height increased modestly at 15 days, with no change in lip volume, and the reported adverse events — temporary numbness, functional limitation — were mild and resolved within 30 days.
That study reports a measurement point, not a duration.
What can be said is the direction, not a figure.
The effect is temporary by how the drug works: the Botox Cosmetic label describes the toxin partially blocking the nerve signal to the muscle, with reinnervation slowly reversing that block.
And because a lip flip uses a small dose, a shorter result than upper-face toxin treatment is a reasonable expectation — a reasoned expectation, not a measured figure, and we found no trial that has tested it.
The "6 to 12 weeks" figure is unverified: it comes from clinic and consumer sites, not from the FDA label or a controlled trial. A number a practice quotes you is that practice's experience, not a tested result — a fair question is how they arrived at it.
What affects how long a lip flip lasts
We found no study that has tested what makes a lip flip wear off sooner or later.
What follows is what toxin research in studied areas shows, plus the flip's own dose data — not lip-flip trial results.
Dose is the clearest duration factor in the toxin data that exists.
In a 64-week Japanese study of 363 people, a 20-unit Botox dose for frown lines lasted longer than a 10-unit dose, with means of 17.1 versus 14.8 weeks — evidence that smaller doses wear off sooner.
The flip's doses are smaller than either: the 2025 review reports 4 to 6 units, and the measured study used 4.
There is no label dose to anchor those numbers, since no label covers the lips, so the amount is the provider's judgment.
See how long Botox lasts for the area-by-area data behind the upper-face figures.
Dose and duration: what has been measured
| Treatment | Dose | Mean duration |
|---|---|---|
| Botox for frown lines | 20 units | 17.1 weeks |
| Botox for frown lines | 10 units | 14.8 weeks |
| Lip flip | 4 to 6 units | Not measured in any study we found |
The product matters too, and it is where quotes get muddled.
Botox Cosmetic's label states its units cannot be compared to or converted into units of any other botulinum toxin product, and the lip-flip studies mostly report onabotulinumtoxinA-equivalent doses without standardizing by brand.
A duration seen with one brand's 4 to 6 units is not automatically a duration for another's.
The person is the third factor.
The duration factors the label and trial data support are dose, muscle strength and individual variation, with sex differences showing up in trials.
What is not supported: the familiar claims that exercise or a "fast metabolism" makes toxin wear off faster — we found no label or controlled trial showing that.
If a flip seems to fade unusually fast for you, the questions to take back to your provider are which product was used and how many units, not your workout schedule.
Lip flip vs lip filler for this question
For the "how long" question in lip flip vs filler, the two treatments sit in different evidentiary positions.
A lip flip relaxes the orbicularis oris and adds no volume; lip filler adds volume.
The flip has no measured duration we could find.
Filler has no single duration either, but it has FDA's own framing: how long a filler lasts depends on the filler material and where it is injected, and absorbable fillers are temporary and may need repeat treatment.
The regulatory picture explains part of the gap.
Filler lip augmentation is an approved use for adults over 21 — FDA regulates fillers as medical device implants — while the flip is off-label for every US toxin brand.
Filler duration is therefore described per material and per injection site; the flip has no product-level duration figure we could find.
Lip flip
- Relaxes the orbicularis oris; adds no volume
- Off-label for every US toxin brand
- No measured duration we could find
Lip filler
- Adds volume
- Lip augmentation is an approved use for adults over 21
- Duration depends on the filler material and injection site
Practically, this means no head-to-head duration comparison can be drawn from published data.
Anyone who tells you a flip outlasts filler, or the reverse, is comparing a measured figure against an unmeasured one.

What to ask your injector
Because we found no tested number, your provider's experience is where any duration estimate will come from — so ask how they build theirs.
Questions worth bringing: what they see in their own patients and when they suggest a repeat treatment; how many units they plan and which toxin they would use, since units are brand-specific; and what their plan is if mouth function is affected afterward — the studies' reported effects, difficulty whistling or drinking through a straw, resolved within weeks.
Two label points frame the touch-up conversation.
No US toxin label recommends a routine two-week touch-up; a two-week follow-up is common clinical practice, not a label requirement.
And the Botox Cosmetic label says the safety and effectiveness of dosing more often than every 3 months have not been clinically evaluated.
For the lips there is no label schedule at all, so the interval between treatments is a question for the provider who treats you, not something this page can set.
All botulinum toxin products carry an FDA boxed warning for distant spread of toxin effect: weakness, double vision, drooping eyelids, or swallowing or breathing difficulty, reported hours to weeks after injection. Swallowing and breathing problems can be life threatening and there have been reports of death. The Medication Guide says to seek immediate medical help for swallowing, speaking or breathing problems.
Find a lip flip provider near you
Whoever you see, the sourcing rules are the same as for any toxin treatment.
CDC advises getting botulinum toxin injections only from a licensed, trained provider using product from an authorized source, and says that even with FDA-approved product given correctly the procedure carries risk.
CDC also notes that your state might have a look-up tool to check whether a provider or setting holds the appropriate license.
Who may inject is state law, not a national rule: AmSpa, a medical spa trade association, says injecting botulinum toxin is the practice of medicine because a prescription drug is used, so it must be done by a physician, an independent-practice nurse practitioner or another licensed professional acting within their scope — under rules that vary by state.
For what the treatment is, what the studies show and what it costs, start with the lip flip guide.
Then bring the duration questions above to the consultation: a provider who can explain how they arrive at their duration estimate, and their units and product choice, is answering from experience you can weigh against other consultations.
Frequently asked questions
Is a lip flip permanent?
No. The flip works by partially blocking the nerve signal to the orbicularis oris, and the Botox Cosmetic label describes that nerve-muscle connection slowly regrowing (reinnervation), which reverses the effect.
The studies report temporary changes only: in the 2026 study, adverse events resolved within 30 days, and the systematic review describes side effects resolving within weeks.
We found no source describing a lasting lip change from a lip flip.
How often can you get a lip flip?
There is no label schedule for the lips: no US neurotoxin label includes them, so the lip flip is off-label.
Two label points frame the spacing question.
The Botox Cosmetic label says the safety and effectiveness of dosing more often than every 3 months have not been clinically evaluated, and no US toxin label recommends a routine two-week touch-up.
Ask the provider who treats you what interval they advise for the dose and product used.
How long does a lip flip take to work?
The controlled data we found is a 2026 prospective study that measured a modest increase in upper lip height 15 days after a 4-unit lip flip in 17 women.
That is when researchers measured a change, not necessarily the first day one was visible.
We found no established onset day for the lips, so ask your provider what timeline their own experience supports.
Why do clinics say a lip flip lasts 6 to 12 weeks?
Because those figures come from clinic and consumer websites.
We found no FDA-label or high-quality trial source for how long a lip flip lasts, so the numbers circulating online are practice estimates rather than tested results.
Because the flip uses a small dose — 4 to 6 units in the published review — a shorter effect than upper-face toxin treatment is a reasonable expectation, but it is not a measured figure.
Does a lip flip or lip filler last longer?
We found no basis for a head-to-head comparison: the flip has no measured duration to put against filler's figures.
FDA says how long a filler lasts depends on the filler material and where it is injected, and that absorbable fillers are temporary and may need repeat treatment.
Anyone quoting a head-to-head number is comparing a measured figure against an unmeasured one, so ask each provider what their own estimate assumes.
